When it fits

Small groups - roughly three to six people - where call volume is genuinely low enough that one person can reasonably carry the pager for a full day without a separate day/night split.

The trade-offs

A 24-hour shift is a long stretch to stay reachable. ACGME caps continuous resident duty at 24 hours (plus up to 4 for handoff) for a reason - this pattern suits low-volume attending call more than a busy residency service.

Pros

  • + Only one shift type to schedule and explain
  • + One clear point of contact at any given moment
  • + Simple to reason about fairness - it's just a count of 24h blocks

Cons

  • Fatigue risk if the night gets busy
  • The group needs enough people that no one takes call too often
  • Not ACGME-compliant for residents without additional protections
