When it fits

Residency programs and services replacing traditional overnight call with a dedicated overnight team for a defined stretch - commonly one to two weeks - while day teams run weekdays and a separate weekend rotation covers the gap.

The trade-offs

Concentrating night work into a block is easier on circadian rhythm than scattered single nights, but can front-load fatigue during the float week itself. Weekend coverage still has to be staffed as a third shift type.

Pros

  • + Fewer people exposed to overnight work at any one time
  • + One circadian shift per float block instead of one per night
  • + Clean separation between day-team and night-team responsibilities

Cons

  • The float team can feel disconnected from daytime continuity of care
  • Float blocks need to rotate fairly across the group over the year
