When it fits
Surgical and specialty groups where weekday call volume is light enough for home call, but weekends need heavier - sometimes in-house - coverage.
The trade-offs
The weekend shift needs a much higher fairness weight than a weeknight - it's a full 24 hours of what would otherwise be someone's weekend. Rotation has to spread weekends deliberately, not just alphabetically.
Pros
- + Matches staffing to real demand - lighter weeknights, heavier weekends
- + Familiar to groups already running home call on weeknights
- + Scales down to smaller groups than a full day/night split needs
Cons
- − Weekend equity is usually the single biggest fairness complaint in this pattern
- − Weeknight home call can still interrupt sleep without counting as a full shift
